Select patient forms are required and collected for the provider prior to an appointment. You can find all the instructions and details for an appointment in your email confirmation.
Visit the Patient Portal to create a new account. Select My Health Option and enter all medications, medical history, and tobacco history. You also have the option to download your already existing medical information.
Next, go to the Medical Forms to complete the authorization for prescriptions, the HIPPA forms, and Patient Registration form.
